Regular in an engine that RECOMMENDS premium

Most modern engines have a knock sensor. Feed a 'premium recommended' engine regular and the computer detects incipient knock and retards ignition timing to protect itself. The car runs fine — you just lose a little power and efficiency. Studies generally find a few percent, not a catastrophe.

This is the common, low-stakes case: it won't hurt the engine, it just slightly underperforms.

Regular in an engine that REQUIRES premium

If the manual says premium is required (often high-compression or turbocharged engines), running regular repeatedly is harder on the engine. The knock sensor still pulls timing, but under load — hills, towing, hard acceleration — sustained knock can cause real wear over time. In a pinch a tank won't destroy it, but it's not a habit these engines are built for.

Premium in an engine that only takes regular

This is the one people waste the most money on. Putting premium in a regular-fuel engine does essentially nothing — no extra power, no better mileage, no meaningful 'cleaning' beyond what the detergent package already does. The engine can't use the extra octane. You're paying more for a number you don't need.

If you want the cleanliness benefit, that comes from Top Tier detergent gas at any octane, not from buying premium.

Will one tank of regular hurt my premium car?

Almost never. The knock sensor protects the engine by pulling timing; you'll just notice slightly less power until you fill with the right grade.

Does premium clean my engine better than regular?

No. Cleaning comes from detergent additives (the Top Tier standard), which are in regular too. Premium octane and detergents are different things.
